This is an update of last year’s column as the maximum and minimum payments for the various disability programs have changed. More details are in the benefits books.
Workers’ Compensation (State law) · After 5 calendar days of disability, retroactive to the 1st day if disability lasts 21 days or more. · “Disability” defined as “unable to work” because of job-related illness or injury. · Must be under a doctor’s care. Benefit pays: · 60% gross average weekly pay (pay calculation includes OT) up to a max of $1043.54 as of 10/07. · Payments are not taxed. · Pays up to 3 years of “total temporary disability.” · “Partial Disability” pays 60% of “lost earning capacity” up to a max of 75% of total disability rate up to 5 years. Includes compensation for not being able to work OT. How Initiate? · GE: Injured workers must notify supervisor and report to GE Medical Center.(GE Guards can do the report if Medical Center is closed.)· Ametek: Notify your supervisor and contact Jim Rose at the Safety Office at EXT. 4479 to file a report. · Veolia: Notify your supervisor and Linda Holland. · Make sure occupational “Injury/Illness Report” is filed at the Medical Center (GE) & Safety Office (Ametek.) & Linda Holland (Veolia.) GE, Ametek & Veolia Short-Term Disability (STD) When Qualify?· Must be under the care of a doctor. “Disability” defined as unable to perform the regular duties of your job. · GE: 8th day disabled, or 1st day of hospitalization or day surgery in an approved ambulatory surgical facility. · Ametek/Veolia: 8th day disabled due to sickness. No waiting period if disabled by an accident or inpatient hospital stay. Benefit Pays: · GE: 60% of weekly pay up to a maximum of $650 as of Jan. 2008. · Ametek: 60% of weekly pay up to a max of $450 as of Jan. 2007. · Veolia: 80% of pay based on service, then reduced to 60%. · Pays until recovery or 26 weeks. · Benefits are taxed. · Will pay if delayed/denied by Workers’ Comp. Reimbursed if you win Comp. How Initiate?· GE: Call GE Disability Center at 1-800-392-0789 once you expect to be absent from work for 7 days or after discharge from hospital or surgical facility. · If call is not made within first 7 days of disability or discharge from hospital, benefits will be reduced by 50% until the call is made. · Ametek: Call Hartford Life at 1-877-534-6206 ASAP or by the 5th day if there’s a waiting period. If a planned procedure, call the 1-800 number in advance for timely benefits. · Veolia: Contact Linda Holland. GE, Ametek & Veolia Long-Term Disability Income (LTDI)When Qualify?· After 26 weeks of Short-Term Disability, or Workers’ Compensation. · Must be enrolled. · “Disability” defined as unable to perform the regular duties of your job for 12 months of disability. (24 months for Veolia.) · After 12 months, “disability” defined as “unable to perform any job for which reasonably suited by means of education, training, or experience.” · Must be under a doctor’s care. Benefit Pays: · GE: 50% of monthly pay capped at $3000 per month as of April 2006. · GE Salary: has option to purchase option that pays 60% or 70% of pay. · GE: guaranteed minimum of $200 as of April 2006. · GE Salary: still has option for $250 and $300 minimum benefit corresponding to option purchased. · Ametek: 60% of pay with $100 minimum. · Veolia: Basic LTD is 50% of pay capped at $7000 per month. Option to purchase additional coverage. · Benefits are not taxed. How Initiate?* GE: Disability Center should coordinate rollover to LTDI. Call 1-800-392-0789 if questions or receiving Workers’ Comp. · Ametek: STD should coordinate rollover to LTDI. Contact Ruth Asher if questions or if on Workers’ Comp. · Veolia: Contact Linda Holland to ensure rollover.
